Juniors Fall League Team Registration Form

Cost: $800 per team
Limit of 12 players per roster
Must have at least 6 players to register a team
All players must be in the 6th grade or above to participate
Junior Varsity teams are limited to 2 players that are playing on a school Varsity team
Registration is not complete until your payment has been made.
By submitting the Juniors Fall League Individual Registration Form you acknowledge the inherent risks involved and forever waive the Old Dominion Volleyball Academy from any and all liabilities as result of your giving permission for your child to participate in this event.
